Notice of additional health check-up schedule for staff who have not had a health check-up yet

1. To facilitate the staff who were busy with work and could not attend the health check-up on May 16, 17, 19, and 20, the School will coordinate with Medlatec Hospital to conduct additional health check-ups for the above staff. The specific plan is as follows:

• 8:00 AM, May 24, 2016 (Tuesday)
• Location: Medlatec Hospital, address 99 Trich Sai, Buoi Ward, Tay Ho, Hanoi (this is the new facility of the hospital).

The bus to take staff for additional medical examination departs from the School at 8:00 a.m. on May 24, 2016.

The Union Office requests that the department union chairman and the department union executive committee notify the staff and register for additional medical examination.before 4:00 p.m. on May 22, 2016 for the Faculty Union to synthesize and send to the School Union.

The School Union Office will not process registrations after the above time.

2. The hospital will return the files to the staff and advise the staff who have not been advised and the staff who need additional examination at 8:30 a.m. on June 2, 2016 (Thursday) at the School Union Office, room 603, building E.

Some officers, after the medical examination, brought their medical examination records home without the consent of the Organizing Committee. We request that the Department's Union Executive Committee notify the above officers to bring their records to the Union Office on May 23, 2016 so that the Hospital can enter the records into the system to save the data.
