10. Summary of new results of the thesis:

The capacity of primary health care is still weak and not uniform in terms of service provision capacity, human resources, information, technical capacity, pharmaceutical technology, financial capacity and management and operation capacity. Survey data shows that 77% of health workers believe that hospitals provide enough medicine; only 37% of health workers believe that there is a match between their training and current job position, only 29% of health workers at district hospitals are university-level trained. 65% of health workers rate the patient reception process as satisfactory. People's assessment of facilities and medical equipment at the commune level is still low. 72% of opinions say that health stations are still at an average level, only 5% rate them as good. The district level is rated better by people, 53% rate them as good. Regarding administrative procedures for medical examination and treatment, they are still mainly at an average level, accounting for 60%, and 20% rated them as not good. Regarding human resources of medical staff, 13.2% of people rated them as good, 54.2% rated them as average, and 32.6% rated them as not good. Pharmaceutical staff are still lacking in both quantity and quality at both health stations and district hospitals. Regarding technical capacity, pharmaceuticals are closely related to human capacity. When there is a shortage of human resources, the ability to provide technical and pharmaceutical products is also poor.

The organizational capacity is both lacking in formal training and weak, which leads to the weak operation of the functions of the primary health care system and many shortcomings. The technical staff at the primary health care level is not enough, there are high-level techniques but there is no suitable human resource to train and use them, the equipment is still lacking compared to the regulations of the Ministry of Health and actual needs.

Although financial capacity has improved, especially at the district level, at the health station, the funding source is still very limited to meet the current functions and tasks of the station. The capacity for service provision still has many shortcomings such as environmental pollution, medical waste treatment, clean water, and behavioral cultures at the grassroots health care level have not been paid attention to and developed. The ability to treat diseases is still weak, so the implementation of the functions of the grassroots health care level still has many difficulties.

There are a number of factors that affect the capacity of primary health care such as dynamic policy systems, organizational management mechanisms, environmental conditions, climate change, local leaders' perspectives, salary and allowance regimes, working conditions, people's evaluation of current primary health care services and people's needs related to medical examination and treatment. People's needs are mostly in the basic needs group such as adequate medical equipment, people can examine and treat at the commune health station where they register for initial medical examination and treatment, correct and accurate medical examination and treatment, building more primary health care facilities closer to the people, being welcomed attentively, medical staff having a gentle attitude and consulting to help patients understand their diseases.

The research results help policymakers, leaders, researchers, health workers, and sociologists to determine appropriate directions and solutions to enhance the role of primary health care in public health care.

10. Summary of the new findings of the thesis:

The capacity of health facilities at grounds level were still weak and uniformity of service provision capacity, human resources, information and technical capacity, pharmaceutical technology, financial capacity and governance capacity. Survey data shows that 77% of medical staffs think that hospital provides adequate of medicines; only 37% of health workers believe that there is a mismatch between professional training and current positions, only 29% of health workers at the district hospital have university degrees. 65% of staffs assess the patient reception procedures are satisfactory. Evaluation of the people on facilities and medical equipment at the health commune level is still low. 72% of respondents said that health communication centers are still inadequate, only 5% rated as good at. District hospital was rated better by people in Ha Trung district, 53% said that is at good level. In terms of administrative procedures in health care and treatment is still largely inadequate accounting for 60% and 20% rated the undergraduate level. Regarding health workers, people rated with 13.2% at good, 54.2% assessed at the average level, 32.6% said it is not good. Lacking of pharmaceutical staff in both quantity and quality in all commune health centers and district hospitals. Capacity on both technical and pharmaceutical aspect have strong relationship with human resources, lack of manpower led to the ability to provide the techniques and pharmaceutical poor also followed.

Organizational capacity and insufficient training and therefore entail major operational functions yet powerful medical facility is inadequate. Technical staff at the health facility at grounds level is not enough. There are highly technical but no appropriate personnel to train and use. Equipment at health facilities is not enough as principle of Ministry of Health and needed in reality.

Financial capacity has been improved but especially at the district level. However, budget at commune health center is still very limited to meet the functions and duties of the current request. Capacity of service delivery has a lot of difficulties of such as environmental pollution, medical waste disposal, and clean water. The behavioral culture at health center at grounds level has not yet been paid attention and strengthened. Ability to treat the disease should remain weak therefore implementations of functional health facility still many challenges.

There are many factors affecting capacity of health facilities at grounds level such as policies, management mechanism, environmental conditions, climate change, factor perspective of local leaders, wages, allowances and evaluation of people about the current service medical facility, as well as the needs of the people related to health care. The needs of people in Ha Trung district are mostly basic needs such as essential medical equipment, people can check health and treatment at commune health center or where people can initial medical examination and easy to access to correct medical treatment and convenient services. In addition, other group of needs such as the attitude and good behavior of the medical staffs and counseling to help patients understand the disease is also important needs that most people are referring to.

Findings help policy makers, leaders, researchers, staff in the health sector; Social studies can be used to determine the direction and the appropriate solutions in order to strengthen the role of the medical facilities in the health care of people.
