Application documents:

• Original University Entrance Examination Results Form 2010 (Form No. 1). Photocopies, even notarized photocopies, will not be accepted.
• The envelope must be stamped and clearly state the candidate's full name, mailing address, and phone number (if available).

* Application fee: 15,000 VND.
