
Detailed instructions for filling out the "National High School Examination and University/College Admission Application Form 2017": Section DEPARTMENT OF EDUCATION AND TRAINING…DEPARTMENT CODE: Candidates registering at a testing center belonging to a specific Department of Education and Training should write the name of that Department in the blank space, then fill in the two digits representing the Department code in the next two blank spaces. The Department of Education and Training code is regulated by the Ministry of Education and Training. Section Form Number: This is filled in by the registration center; candidates should not fill in this section.
Sections 1 and 2:Fill out the form according to the instructions on the National High School Examination and University/College Admission Application Form (hereinafter referred to as the Application Form).
Section 3:a) For the candidate's place of birth, simply state the name of the province or city. If born abroad, only state the name of the country (in Vietnamese). b) State the ethnicity exactly as stated on the birth certificate.
Section 4:For the old-style National Identity Card, write the 9 digits in the last 9 boxes on the right, leaving the first three boxes blank; for the new-style National Identity Card or Citizen Identification Card, write all 12 digits in the corresponding boxes.
Section 5:The provincial (city), district (county), and commune (ward) codes for communes (wards) classified as particularly disadvantaged will be determined by the Ministry of Education and Training. Candidates should check at their registration location to correctly enter the provincial (city), district (county), and commune (ward) codes corresponding to their current permanent residence into the respective boxes on the right. Candidates who do not have permanent residence in a particularly disadvantaged commune should leave the commune code box blank. After filling in all the administrative unit codes, candidates should clearly write the name of the province (city), district (county), and commune (ward) in the blank line. For candidates eligible for priority based on their permanent residence, they must confirm that they have had permanent residence for more than 18 months in Area 1 or more than 18 months in a particularly disadvantaged commune during their high school studies by marking the corresponding box.
Section 6:Write the school name and address up to the district (county) and province (city) on the dotted line. Write the province code where the school is located in the first two boxes, and the school code in the next three boxes (the school code is written according to the regulations of the Department of Education and Training; if the school code has one digit, the first two boxes should be 0; if the school code has two digits, the first box should be 0). For candidates who are police officers or military personnel sent to participate in the university/college entrance exam, write the province code corresponding to the province where they are stationed and the high school code is 900. For candidates who have studied abroad, write the province code corresponding to the province of their permanent residence in Vietnam and the high school code is 800. In the class name section: clearly state the name of the 12th grade class the student is currently studying in (e.g., 12A1, 12A2,...). For independent candidates, write "TDO".
Section 7:Please provide your phone number and email address (if any). For candidates who wish to adjust their online application, they must register their personal mobile phone number to receive a one-time password (OTP) via SMS, ensuring security during the online application process.
Section 8:Candidates must clearly state the full name of the contact person and detailed address: hamlet (house number), village (street, alley), commune (ward), district (county), province (city). This address will also be the address to receive the Admission Notice if the candidate is admitted.
Section 9:Candidates who wish to use their exam results for admission to universities and colleges in the teacher training field should mark (X) in the box next to it.
Section 10:Candidates must mark (X) in one of the two boxes to clearly indicate whether they are studying under the high school program or the continuing education program.
Section 11:For independent candidates, mark (X) in one of the two boxes to clearly distinguish whether the candidate is an independent candidate who has not yet graduated from high school or has already graduated from high school (as of the time of the exam).
Section 12:Candidates registering to take the exam at a specific examination center should write the name and code of the examination center as prescribed by the Ministry of Education and Training in the corresponding position.
Section 13:Students currently in 12th grade at a high school should submit their application at that school. Other applicants should submit their applications at locations designated by the Department of Education and Training. The registration unit code should be filled in according to the instructions provided by the registration center.
Section 14:For candidates currently in 12th grade (not yet graduated from high school), they must register for the exam at point a; they are not allowed to choose individual subjects at point b. For independent candidates, depending on their purpose for taking the exam and their choice of subject combinations for university/college admission, they may choose the entire exam (at point a) or only select some individual subjects (at point b) as appropriate. In the case of independent candidates who have not yet graduated from high school and have subjects (for graduation recognition) from the previous year that meet the requirements for retention, if they wish to retain any exam/subject component, they must record the score for that exam/subject in Section 15; however, candidates can still choose to take the exam/subject component (that has been retained) to use the results for university/college admission. For candidates studying under the Continuing Education program, they may choose a foreign language exam if they wish to use a foreign language in their subject combination for university/college admission. How to select the component test/subject: Candidates should mark (X) in the box corresponding to the component test/subject they wish to take. For the Foreign Language test, candidates should fill in the code corresponding to the specific language as follows: N1 – English; N2 – Russian; N3 – French; N4 – Chinese; N5 – German; N6 – Japanese.
Section 15:For candidates applying for exemption from the foreign language exam, they must clearly state the type of certificate that qualifies them for exemption or state that they are members of the national team participating in the International Foreign Language Olympiad as regulated by the Ministry of Education and Training.
Section 16:Candidates who took the high school graduation exam last year, if they have subjects that qualify for grade retention according to regulations, should enter the scores for those subjects in the corresponding box. For subjects that are retained, candidates can still register to take the exam (either the full exam or individual subjects) in Section 13 if they wish to use the exam results for university or college admissions.
Section 17:Candidates must identify their priority group themselves and correctly record the symbols for priority groups as stipulated in the Regulations on University and College Admissions for Teacher Training Programs or related guiding documents. False declarations will be processed according to current regulations. Candidates eligible for priority must submit all legally valid supporting documents to the school upon enrollment.
Section 18:For candidates applying to universities and colleges, please fill in the area code in the blank space as follows: Area 1 (KV1) enter the digit 1, Rural Area 2 (KV2-NT) enter 2NT, Area 2 (KV2) enter the digit 2, Area 3 (KV3) enter the digit 3. During the three years of high school or equivalent, the area where you studied for the longest period will determine your regional priority. If you studied at a different school with different regional priority levels each year, or spent half your time at one school and half at another, the area where you graduated from high school will determine your regional priority. For candidates entitled to priority based on their permanent residence, please refer to the regulations of the University and College Admission Regulations for teacher training programs and the guidelines of the Ministry of Education and Training to correctly fill in the area of priority you are entitled to. The exam registration software will automatically determine the level of regional priority you are entitled to according to the University and College Admission Regulations for teacher training programs. For admission to regular teacher training programs at colleges, candidates must fully complete Sections 5 and 6; if the information differs from what was declared on the application form, candidates should double-check the information provided in these sections.
Section 19:Fill in the information as instructed on the registration form.
Section 20:For candidates taking the exam with the aim of using the results for admission to college or university, please mark (X) in the box corresponding to the level of education you have graduated from: Graduated from vocational school (TC) or Graduated from college (CĐ).
Section 21This section is for candidates wishing to apply for university admission; and college admission for teacher training programs. To fill in this section, candidates need to carefully research the admission information of their desired school, which is published on the Ministry of Education and Training's admissions portal and the schools' websites, to ensure accurate registration information regarding school code, major/group code, major/group name, and subject combinations for admission. Any application that does not comply with the schools' regulations will not be entered into the software system for consideration.
Note:Candidates must fill in all required sections completely, clearly, and neatly, without making any corrections or erasures. If it is a number, write it using Arabic numerals (0, 1, 2, 3…), not Roman numerals (I, V, X,…).
