The thesis has clarified the following issues:
- The topic has clarified the current status of activities, goals, and objectives of the community-based elderly health care and consultation model in Hop Hung commune, Vu Ban district, Nam Dinh province based on the two main activities of the model: self-consultation, physical health care and counseling, mental health care.
- Based on the analysis of the current status of the community-based elderly health care and consultation model, it is possible to see the factors affecting the model's activities.
- Finally, the study evaluates the performance of the model and provides policy and practical recommendations for better intervention and support for the community-based elderly health care and consultation model in general and the elderly in particular.

Thesis was to clarify the following issues:
