10. Summary of thesis results:
The study was conducted to present the current status of primary health care programs for rural women in Thanh Ha commune, Thanh Liem, Ha Nam; evaluate the effectiveness and at the same time point out the advantages and difficulties leading to that result; analyze the factors that directly affect primary health care work including the institutional system, the team of commune health workers and the policies being applied to local women. In the research content, the author also tried to understand and analyze the impact from stakeholders such as local authorities, families and other individuals in the community.
The results of the study show that the work of primary health care is still limited, although programs for women in the commune exist and meet part of the needs of the people. This situation comes from the fact that the content of the programs has not shown diversity, Thanh Ha commune has only focused on providing health services but has not invested in propaganda and education activities and activities with community participation to raise awareness of primary health care for women. In addition, the study has clarified the strengths and barriers of the community, thereby finding the potential and resources that can participate in improving the effectiveness of primary health care programs. It is noteworthy that the results from the questionnaire method, in-depth interviews and observations have shown that women in the Thanh Ha commune community are the most valuable resource, they can bring both spiritual and material support.
From the results of this study, the author proposed community-based interventions to improve the effectiveness of primary health care for women in Thanh Ha commune. From a social work perspective, the study did not propose interventions to solve medical difficulties but focused on improving and supplementing community activities and promoting the role of women in the content of the programs. In addition, the study also mentioned the role of social workers in the intervention process.
11. Practical application:
The community-based intervention model proposed in the study can be applied to rural areas with similar conditions to Thanh Ha commune to improve the effectiveness of primary health care for women. Furthermore, the study will also contribute data for the development of community-based intervention models of the social work sector in the field of community health care.

10. Summary of the findings of the thesis:
The study is carried out to give the status of the health care program for the rural women in Thanh Ha commune, Thanh Liem, Ha Nam; assess the efficiency and at the same time point out the advantages and disadvantages that result; analyze the factors that impact directly to the PHC system includes institutions, medical staff and social policies that being applied to local women. In research content, the author also attempts to understand and analyze the impact of the stakeholders including local governments, families and other individuals in the community.
Results of the study show that the PHC has limitations, althought programs for women are existing society and meet the demand part of the people, this situation stems from the content of the programs has not shown the diversity, Thanh Ha just emphasized on providing health services without investing in education and communication activities and events with the participation of the community to raise awareness of health care for women. In addition, research has illuminated the strengths and barriers that community from realizing its potential and resources that could be involved in improving the efficiency of the primary health care program. It is noted that the results from the questionnaire method, in-depth interviews and observations have shown that the women in the community Thanh Ha are the most valuable resource, they can bring both helps of spiritual and material.
From the results of this study, the author has proposed ways of community-based interventions to enhance effective primary health care for women Thanh Ha. Standpoint of social work, research does not provide interventions to address these difficulties by focusing medically that focus on improving and adding community activities and promoting the role of women in the program content. Besides, the study also mentioned the role of social workers in the intervention process.

Form of community-based intervention, is given in the study, can be applied to the rural areas that have similar conditions Thanh Ha commune to enhance effectiveness of the primary health care for women. Furthermore, the research will also contribute literature to develop intervention model towards community development of the social work sector in the field of public health care
